Daleville Family Counseling is an exclusive telehealth provider. All sessions will be conducted virtually.

Complete the registration below
- Complete the online registration form below in its entirety.
- Be sure to read and sign the Informed Consent HIPAA Policy at the bottom of the form.
- Submit the completed form.
2.
Review your insurance coverage
- Obtain your insurance information to avoid any unexpected fees.
- Call your insurance company to ask if your deductible applies to outpatient mental health counseling sessions.
- If you are unsure, you should also find out what your co-pay or co-insurance costs may be.
3.
Book your first session with Dee
- Once your form has been submitted and received, Dee will contact you personally by phone or by email to schedule your first session.
- Daleville Family Counseling is open Monday through Friday from 9am to 5pm.
NOTE: As Dee is licensed exclusively in the state of Virginia, any client of DFC must be physically present in the state of Virginia during telehealth sessions. This policy is in compliance with most insurance companies’ telehealth coverage requirements.
