Terms to Receive Correspondence Services
Upon request, I will file any needed correspondence on a client’s behalf. Prior to my releasing any information to anyone about a client, an AUTHORIZATION TO RELEASE INFORMATION must be completed and submitted. Additionally, the mandated correspondence fee must be paid in advance in order for any documentation to be released by Daleville Family Counseling on a client’s behalf. Additionally, any outstanding balance on the requesting client’s account must be paid in full before correspondence is requested. Once these terms are met, I will release your records or documentation to fulfill your request. Fees associated with this service are assessed based on the amount of time that is needed to fulfill your request.
No Show & Late Cancellation Fee Policies
Clients who do not show up for a scheduled appointment with Dee will be charged a no-show fee of $100. “No-shows” are only permitted 2 times before future sessions are discontinued.
Clients are required to make changes or cancellations to their appointments at least 24 hours in advance of their appointment to avoid being charged a $100 late cancellation fee. The exception to this policy is a same-day illness or emergency, in which case the fee is not charged so long as the client contacts Dee prior to the appointment.
If clients do not cancel or change appointments within the timeframe(s) required, a late-cancellation fee of $100 will be charged to the client. Insurance does not cover fees due to late cancellations or no-shows.
